Vaccine Information

COVID-19 vaccine appointments are now handled by the health department's regular immunization clinic located at 301 W. Alder St. Appointments are primarily offered on an appointment only basis. These are one-on-one appointments in a clinical setting with a nurse. MCCHD no longer offers mass vaccination style services of any kind.

NEW: Updated (Bivalent) boosters now available for ages 6 months and older. 

This updated booster protects against the original and newer Omicron strains. Some children may not be eligible for bivalent boosters depending on the vaccine schedule they have followed*. Call 406-258-3363 to see if your child is eligible/book your appointment.

*Children 6 months - 4 years who have had 3 pediatric Pfizer doses are ineligible for bivalent boosters as the CDC has noted their 3 doses still provide ample protection at this time.

Appointment Paperwork

To ensure a quick experience, print and fill out this registration form, this screening questionnaire and this Covid screening form. Bring each to your appointment. Those who cannot print this form and fill it out ahead of time will be able to fill it out at the clinic.

In accordance with the ADA, Missoula County will provide reasonable accommodation for individuals with disabilities. To request an accommodation during your appointment, call 406-258-3363, leave a message and we'll call back as soon as we can.

Minor Appointment Paperwork

A parent or guardian should print and fill out the form above for their child in addition to the following forms if applicable. Parents or guardians must also read this Pfizer fact sheet (for 12 and up) or this Pfizer fact sheet (for ages 5-11).

Sixteen and 17-year-olds are eligible for unaccompanied minor appointments. They must bring this unaccompanied minor document, filled out by a parent/guardian, in addition to the documents above.

Parents and guardians can send their child to an appointment with an authorized adult, such as a babysitter or grandparent, if they fill out this third party authorization form.

The health department's immunization clinic is located at 301 W. Alder Street in downtown Missoula between N. Orange St. and Woody St.  Parking is available in front of the building.

Replacement Vaccine Cards

The Missoula City-County Health Department can help individuals replace their vaccine card if they opted into Montana's IMMTRAX vaccination record system when they signed up to get vaccinated. Our staff can assist you in finding your vaccine record and getting you a new card if you call 406-258-3363. You may also walk into the clinic during regular hours and our staff will be able to replace your card right there.
